Checking of Forms Season.
Ano po ba ang sinasabi ng DepEd Order No. 11, s. 2018 - Guidelines on the Preparation and Checking of School Forms?

1. Only School and Division levels checking is allowed.
2. There will be NO District Level checking of forms.
3. Only SF1, SF4, SF5 and SF10 will be subjected to checking.
4. NO additional forms shall be required for checking.
5. Only LIS Generated forms are acceptable. Commercialized forms is not allowed.
6. Source of Funds: School MOOE or school canteen funds for SCC and SDO MOOE for DCC.

Teachers contribution is prohibited

1. LIS System Admin/Coordinator ka ba? sa ayaw at gusto ng SH mo, ikaw ay naka assigned na vice chair ng SCC -DO 11, s.2018.
2. Placing of With Honors, With Honors and With Highest Honors in SF 5 will be in the Column Action Taken (consistent with TE tagging) Kung naka print na, handwritten will do. Remember the instruction from DO11,2018 "Focus on Accuracy instead of cosmetic"
3. Rejected SFs dahil hindi naka computerprinted ang name at ph.d ng mag rereview? "Focus on Accuracy instead of Cosmetic"-DO11,2018
4. Originating School still demanding written request kahit may notice na ng transfer sa LIS? Pls let us know -DO54,s2016/D058.s2017
5. Mandatory ba sa checking ang SF9? No. SF9 will be checked only if SF5 vis-a-vis SF10 not matched. DO 11, 2018
6. SF 10 ng grades 2 and 8 computer encoded or handwritten? normally handwritten, but if kaya i shoot sa printer, ok din. DO58,2017
7. List of Graduates and Ranking of Honors are no longer required to prepare -DO 11,s.2018 page 14
8. LIS/ICT coor as VC ng SCC to sign SFs? Isa sa reason, to ensure na hindi galing sa commercialized software ang mga SFs- DO11,2018
9. DO 58,2017 "Original SF10 shall be used". bakit gagawa ng New SF10 ang grades 2/8? ituloy na lang ang ginamit sa Grades 1/7, -SFRT
10. Pwede bang dagdagan ang mga forms na i check sa reading of forms? NO, The policy is clear "No additional Forms/reports"- DO11,2018
