LIS (ALS)
ALS BOCY CY 2018 encoding is now open.
Instruction: All learners attending learning sessions from January 2018 shall be encoded in the system.
Reminder for the Facilitator:
- Facilitator must have finalized his/her CLS for 2017 to be able to access the CY2018.
- Learner with no Status (complete/not complete) must update first to be able to enroll
Reminder for ALS system Admin:
- Update the list of Facilitators and place of assignment.
- Provide technical assistant to facilitators thru managing their accounts that includes but not limited to resetting password
- Coordinate with the division system admin for user access management system (UAMS) concerns.

LIS (K-12)
The following facilities are now available:
- Enrolment of balik-aral learners
- Class Removal for SHS
- Transfer Facility for SHS
- Updating of 1st semester status
- Correction of learner profiles with currrent enrolment for SHS
- Correction of Grade Level/EOSY (K-10)
Soon to be deployed:
- Enrolment of 2nd semester
- Resolution of transfer disputes

LIS (School Forms)
Pursuant to DepEd Order No. 58, s. 2017 (Adoption of New School Forms for Kindergarten, Senior High School, Alternative Learning System, Health and Nutrition and Standardization of Permanent Records), electronic copies of the following school forms are now available:

- Alternative Learning System (ALS) Forms (AF1 to AF5)
- School Form 5: Report on Promotion and Level of Proficiency for Kinder (SF5K)
- School Form 8: Learner’s Basic Health and Nutrition Report (SF8)
- School Form 10: Learner’s Permanent Academic Record for Elementary School (SF10)
- School Form 10: Learner’s Permanent Academic Record for Junior High School (SF10)
- Senior High School Forms (SHSF-1 to SHSF-7, SF8-SHS)
- School Form 10: Senior High School Student Permanent Record (SF10-SHS)
WARNING!
It has come to our attention that so-called “electronic school forms” are being sold to public schools.
Please be reminded that DepEd has authorized NEITHER the usage of such “forms” NORtheir sale to our schools.
School forms generated using this “solution” are considered NULL AND VOID, and will NOT be considered for official transactions.
Official School Forms can be downloaded for FREE and are EXCLUSIVELY available for schools at the Learner Information System (LIS).
SCHOOL FORMS ARE NOT FOR SALE.**
Report any incident of unauthorized use and sale of School Forms to the LIS/EBEIS Helpdesk, the Planning Service, and the Office of the Undersecretary for Planning and Field Operations.
